Scripps Clinic offers a one-year Mohs Skin Cancer/ Procedural Dermatology/ Dermatologic Surgery Fellowship under the guidelines and with the approval of the American College of Mohs Micrographic Surgery and Cutaneous Oncology continuously since 1984. ACGME approval was granted in April, 2004 effective July, 2004. Scripps Clinic employs multiple dermatologists in addition to Dr. Greenway, Dr. Daniel Zelac, and Dr. Seaver Soon, in the Mohs Surgery Unit, and offers a full range of clinical and research specialists. Multiple satellite locations as well as local and regional physicians refer patients for specialized care. Other fellowship programs also exist in various departments at Scripps Clinic, each coordinated by the Department of Graduate Medical Education which handles our Internal Medicine Residency.

Completion of an approved Dermatology Residency with board certification or eligibility is required. Duration of training will be one year normally to begin July 1st. A California license is required, as well as BLS certification. Advanced Life Support certification is required to complete the fellowship.

Cases for Mohs Surgery will be performed under the supervision of Dr. Greenway, or other approved Mohs Surgery faculty. Repairs will be done with Dr. Greenway or other supervising staff (dermatology, plastic surgery, oculoplastic, head and neck surgery, etc.). New patient and follow up clinics exist as do consultant staff radiotherapy, maxillofacial prosthetics, oral surgery, hematology/oncology, and others.

Adequate training in anatomy will be provided via a fresh cadaver laboratory. The fellow will learn the operation of the Mohs laboratory and become proficient at preparing sections in both the fresh and fixed techniques. All sections will be reviewed with faculty Mohs surgeons. Attendance at scheduled didactic sessions (Grand Rounds, formal lectures, etc.) are an integral part of the program. Dermatopathologist consultation is available at all times.

The fellow may also participate in other aspects of dermatologic surgery. This will include, but is not limited to, hair transplants, dermabrasions, chemical peels, scar revisions, cosmetic procedures, podiatric procedures, particle resurfacing, various aspects of cutaneous laser surgery (including CO2 and/or Erbium Laser skin resurfacing, plus other lasers, i.e. vascular, pigmented, hair removal, and other systems) under the direct supervision of Dr. Victor Ross, and tumescent liposuction. Additionally, the fellow may participate in the teaching program of basic and advanced surgery to dermatology residents of the Department of Dermatology at the Naval Medical Center, San Diego, and the University of California, San Diego. He or she may also participate in the one week “Superficial Anatomy and Cutaneous Surgery” program for practicing dermatologists presented in conjunction with the University of California, San Diego.

The fellow will become proficient in the application of all types of bandages required for dermatologic surgery and Mohs Surgery. This often will require time on weekends and holidays for patient bandage changes. The fellow will carry a beeper and be on call for emergencies. Staff back up will be provided at all times.

As Mohs surgeons are expected to be clinical skin oncologists, the fellow will assist in running the monthly Visual Tumor Board.

One half day per week may be devoted to research/library time. A minimum of two published papers are required during the fellowship. It is anticipated that the fellow will participate in research projects during the fellowship.

Attendance at the annual meeting of the American College of Mohs Micrographic Surgery and Cutaneous Oncology is recommended. The fellow is encouraged to attend other meetings throughout the year. The fellow will participate in our annual “Melanoma Update” CME course held each January.

The fellow will function as an integral member of the Mohs surgical team, as a precise team approach is required in all cases. Schedules, patient load, and long hours may become demanding, but will provide all the opportunities for the fellow to spend a successful and rewarding year. The fellowship program complies with the resident duty hours restrictions of the ACGME. The Program Director closely monitors fellow duty hours and time management.

Completion of the fellowship allows the fellow to qualify for membership in the American College of Mohs Micrographic Surgery and Cutaneous Oncology.

Scripps Clinic will continue the Fellowship association/approval first established in 1984 with the American College of Mohs Micrographic Surgery and Cutaneous Oncology (Mohs College). Dr. Greenway served as President of the Mohs College in 2000-2002 and was instrumental in the ACGME accreditation process. ACGME approval of the Procedural Dermatology Fellowship at Scripps Clinic was granted in April, 2004 (Effective July 1, 2004). Other than the official name change and added documentation, no significant changes in our outstanding fellowship have resulted from this process.

Following Dr. Greenway’s 2 ½ year tour as CEO of Scripps Clinic, Dr. Greenway became Executive Vice President of the Scripps Foundation for Medicine and Science, on March 1, 2003. Despite the commitment to this system wide role, Dr. Greenway continues to have an active practice, which allows him to be in the Mohs/ Dermatologic Surgery Clinic two and one half days per week. This permits him to have direct supervision and involvement with the fellowship program requirements necessary for completion, and also allows an expanded training base with the other faculty. Dr. Greenway is a Board Member of the American Academy of Dermatology 2004-2008, and is active in a number of national organizations.
