At Scripps Mercy Hospital, our registered nurse case managers assist patients and their families with discharge planning, including placement at various levels of care (e.g., acute rehabilitation, skilled nursing, hospice, assisted living or arrangements for care at home). They also assist with legal or financial issues that may impact the planning process. Our social workers assist patients with crisis intervention, adult and child protective issues and substance abuse-related problems. We also provide a wide range of community resource referrals.
