Scripps and San Diego County Partner to Reduce Hospital Readmissions

Project focuses on improving patient outcomes and cost-savings

Scripps is part of an innovative countywide health care partnership that the Centers for Medicaid & Medicare Services (CMS) just announced it has chosen to receive Medicare reimbursements over the next two years. That means the program can expand its efforts to improve patient care so fewer patients have to be readmitted to the hospital. One of the ways Scripps is working to reduce readmissions is by providing a “navigator” for patients to help them during their hospital stay and to support them after they’ve left the hospital, through home visits and phone calls to make sure their recovery goes well.
