A new Special Needs Plan for people with heart disease is among plan offerings

SCAN Health Plan and Scripps Health have launched an exclusive partnership to offer Medicare beneficiaries in San Diego County three health plan options for 2015.


SCAN members have had access to Scripps’ network of physicians and hospitals since SCAN first entered the San Diego market in 2007. Now, the two organizations have partnered to offer benefit plans expressly designed to serve the growing senior population.

All three benefit plans being offered through the SCAN-Scripps partnership provide all of the basic benefits traditionally covered under Medicare plus a prescription drug benefit (Medicare Part D), with no deductibles and low co-pays for most services. Plan highlights include:


Scripps Classic offered by SCAN Health Plan (HMO) (formerly known as SCAN Classic (HMO)) is a Medicare Advantage plan that is available with no monthly premium. Other benefits include vision, routine transportation, hearing aids, routine chiropractic and acupuncture.


Scripps Signature offered by SCAN Health Plan (HMO) (formerly known as SCAN Signature (HMO)) is a Medicare Advantage plan that is available with a $68 monthly premium. Other benefits include vision, routine transportation, and home delivered meals (based on eligibility).


Scripps Heart First offered by SCAN Health Plan (HMO SNP) is a special needs plan for people with cardiovascular disease or congestive heart failure. The plan offers many of the same benefits as the Scripps Classic plan, as well as additional benefits specific to managing heart disease.


Today there are almost 12,000 SCAN members in San Diego, all served by Scripps’ extensive network of physicians, hospitals, ambulatory facilities, home health, hospice and other community-based programs.

The annual enrollment period for selecting a Medicare Advantage plan for 2015 runs from Oct. 15 to Dec. 7, 2014. During this time seniors and others on Medicare may choose to switch their health plan or move from traditional Medicare to a Medicare Advantage plan (or move back to Original Medicare) with their new elections taking effect Jan. 1, 2015.


As governed by the Centers for Medicare and Medicaid Services, member benefits vary by county. Specific information on SCAN’s 2015 benefit plans is available by calling 855-561-7226, 8 a.m. to 8 p.m., seven days a week. TTY/TDD users may call 711.


SCAN Health Plan is an HMO plan with a Medicare contract offered in California by SCAN Health Plan and in Arizona by SCAN Health Plan Arizona. Enrollment in SCAN Health Plan depends on contract renewal. Heart First (HMO SNP) is available to anyone with Medicare who has been diagnosed with certain cardiovascular conditions.
