A member of the Department of Family and General Practice, Dr. Kozak received his undergraduate degree in biology from the University of San Diego and his medical degree from the University of California Irvine School of Medicine. He completed a residency in family medicine at Ventura County Medical Center, where he served as assistant chief resident. Board certified in family practice, Dr. Kozak has practiced emergency and family medicine throughout California since 1994 and serves as a volunteer physician for homeless clinics. He has received numerous honors and awards including the Role Model of the Year Award from the Ventura County Family Residency Program.
